- Wraparound Facilitation
Leads the wraparound process and ensures meetings remain family-driven and goal-focused. Supports collaboration across all involved systems. - Child & Family Team (CFT) Meetings
Regular meetings that bring together family members, providers, and natural supports. Focuses on planning, progress review, and shared decision-making. - Interagency Coordination
Aligns services across child welfare, mental health, education, and community systems. Reduces duplication and gaps in care. - Service Linkage & Coordination
Connects families to appropriate services and ensures timely access. Monitors follow-through and service effectiveness. - Natural Supports Development
Identifies and strengthens informal supports such as extended family, mentors, and community connections. Promotes sustainability beyond program completion.
