Cornerstone Youth and Family Services provides family-centered wraparound services for youth and families who need individualized support to thrive in their homes and communities. Our programs focus on keeping children in permanent homes and strengthening family stability through coordinated, strengths-based care. We partner with families, agencies, and community resources to ensure every child and family receives compassionate care tailored to their unique needs.

Who Is Eligible?

Families may be eligible for Wraparound services if they are referred by County Children’s Services Departments, connected through county adoption programs, or caring for children in biological, adoptive, kinship, or foster homes.

Referral Process

Our referral process is designed to connect families quickly and efficiently. A referral can be submitted by a child welfare agency, adoption program, or approved partner. Within 24-48 hours, our team reaches out to gather information and begin engagement. We then conduct an initial assessment to understand the family’s strengths, needs, and goals, which guides the development of a customized, family-driven plan of care.

How Wraparound Works

Each family is assigned a dedicated team that may include a facilitator who guides the process and coordinates services, a parent partner who provides guidance from lived experience, and a clinician if mental health support is needed. Families work with their team to develop a family vision that drives the plan, while the team supports the family’s goals and ensures they remain in charge of their successes. Mental health services such as therapy, family therapy, or Intensive Home-Based Services (IHBS) are provided as needed. Child-Family Team Meetings are held regularly to monitor progress and adjust supports.

Wraparound services focus on building trust, cooperation, and family unity through individualized, culturally relevant plans. Our approach creates natural supports to maintain stability beyond program graduation, keeping families together and children in safe, permanent homes rather than in STRTP (short term residential therapeutic placement) most people don’t know what that means. Either put that what it means or say rather than in an out of home placement.
